a. S.S.C. certificate (Proof of date of birth).
b. Registration of PG degree with A.P. Medical Council/Dental Council.
c. Marks lists of Post-graduation (MD/MS/DNB/MDS).
d. Degree/Provisional Degree Certificates(MBBS/BDS & MD/MS/DNB/DM/MCh/MDS).
e. Study Certificate from 4th to 10th Class and Migration certificate, if necessary.
f. Social Status Certificate.
g. PH Certificate.
h. Aadhar Card. - Submit application form online from 07/03/2025 to 22/03/2025.
